Is your drivers license and driving permit expired after having accumulated all its years since it was issued? You need to download and or pick the Uganda Revenue Authority renewal application form for your document.

The forms as seen below can be got from any URA branch or from online like you see. It should be filled and addresses to the licensing officer from where you are handing over the documents.

It should be accompanies by your expired drivers license, a medical signed form and a bank payment.
